Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling on or into protruding rebar. One feasible and acceptable method to correct this hazard is to place protective caps on the ends of the rebar. A) LOCATION: BORED SECTION OF TUNNEL EMPLOYEES WERE EXPOSED TO PROTRUDING REBAR WHEN STANDING ON THE END PLATFORM OF THE TBM DURING BORING OPERATIONS. THE REBAR, PLACED THERE FOR CABLE SUPPORT, WAS LOCATED APPROXIMATELY 5 FT ABOVE THE PLATFORM, PRESENTING A POTENTIAL EYE AND FACE HAZARD IF ONE WERE TO FALL OR WALK INTO THEM. IN ADDITION, IN SEVERAL OTHER AREAS ALONG THE FLOOR OF THE TUNNEL, THERE WERE 2 FT SECTIONS OF REBAR STICKING UP FOR CABLE SUPPORT AND THEY WERE NOT CAPPED OR OTHERWISE PROTECTED.
