Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries resulting from forklift truck tip over accidents: (a) On or about September 9, 1998, the employer did not enforce the use of seat belts which were provided on a 60 Caterpillar forklift. (b) On or about September 9, 1998, the employer had not provided seatbelts on a Model DF8 Nasco Loader forklift. Among other methods, one feasible and acceptable abatement method would be to provide seatbelts and require fork truck operators to use seat belts in accordance with ASME/ANSI B56.1a-1989 Addenda to ASME/ANSI B56.1-1988 and ASME/ANSI B56.1-1993. PLEASE NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
