Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: On or about 12/02/97 employees erecting a steel beam between columns C & D were exposed to the hazards of falls or being struck by falling beam due to the lack of temporary bracing or other securing means during the erection process. Employees were in the process of making a double connection without ensuring the beam was secured and supported. Among other methods, feasible and acceptable abatement methods to correct this hazard include, but are not limited to: Establishment of work practice which assure the stability of the structure through the use of adequate temporary bracing, or other means where needed such as: (1) Support the load by the crane. (2) Shore post the beam from the ground level. (3) Puddle weld or welding clips. (4) Use a come-a- long to secure the beam to the column until the connection is made. (5) Instruct employees not to work under a load. (6) Consult the structural detailer and fabricator concerning the provisions for a seat lug or flange length extension on one of the beams and a corresponding bolt hole in the web of the column as required by ANSI A10.13 1989 section 9.5. PLEASE NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
