Text For Citation: 01 Item/Group: 001 Hazard: HEAT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazard of fire and explosion. Specifically: On July 08, 1996 at approximately 8:45 PM at the No. 5 Hydrotreater a 10 inch stainless steel pipe between heat exchangers X-1103 and X-1100 ruptured releasing hydrocarbons causing a fire and explosions. The 10 inch pipe failed as a result of chloride stress corrosion cracking and metal fatigue which was caused by the piping not being properly passivated during startup activities. Among other methods, one feasible and acceptable abatement method to correct this hazard is to passivate all austenitic stainless steel piping in the No. 5 Hydrotreater using a mixture of soda ash and sodium nitrate. Also, develop and implement a program for periodic inspections using the proper testing to detect chloride stress corrosion cracking in the No. 5 Hydrotreater and all similar units.
