Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that are causing or likely to cause death or serious physical harm to his employees in that an employee was exposed to crushing by a machine that did not have its rear wheels chocked and had jack stands inappropriately placed. a) Track 17, North High Bay Area, Bldg 1, 4001 Vandever Avenue, Wilmington, DE 19802. Employees failed to position wheel chocks on the rear wheels of a Portec RMC Rail Position Machine, allowing the machine to roll backwards, slide off its jack stands, pinning an employee (who was working under the front end of the machine) between the 9000 pound machine and the concrete floor, causing fatal injuries to the employee. Employees had never been trained on the proper placement of wheel chocks and jack stands. 08/17/01
