Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the hazard of equipment failure, in that the employer did not institute a policy and procedure for conducting periodic and regular inspections of horizontal injection molding machines, as evidenced by the amount of oil leaking from the machines in the plant, an employee being caught in the inadequately guarded point of operation or coming in contact with the unguarded heater barrel on horizontal injection molding machines, provide adequate training to ensure the original and continuing competence of personnel caring for, setting up, inspecting and maintaining horizontal injection molding machines and ensuring that operators and supervisory personnel were trained and instructed in the safe method of operation of the safety devices and procedures. Feasible means of abatement include: (a) Insure that all guards are properly installed and maintained on Horizontal Injection Molding Machines used in the facility. Windows in point of operation guards need to be intact and fully secured and heater barrel guards need to be installed in accordance with ANSI Standard B151.1-1997 section 6.3.1. (b) Establish a complete program of periodic and regular inspections of Horizontal Injection Molding Machines in accordance with ANSI standard B151.1-1997 section 4.3. (c) Provide training to ensure the original and continuing competency of personnel caring for, setting up, inspecting and maintaining Horizontal Injection Molding Machines in accordance with ANSI B151.1-1997 section 4.2. Ensure operators and supervisors re trained and instructed in the safe method of operation of the safety devices and operations as described in ANSI B151.1-1997 section 7.1.
