Section 5(a)1: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. On or about February 3, 1997, the Louisville 28 foot (fiberglass) extension ladder Model # FE 3228 was used to gain access to the top of tank #38 located in the compounding department, and was not secured against displacement. "Among other methods, one feasible and acceptable abatement method to correct this hazard would be to secure the ladder at the top in accord with the manufacturer's recommendations, as well as ANSI 14.5 - 1992."
