Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to material handling hoist that were not inspected for defects on frequent and periodic basis: (a)Locomotive Component Shop (south Side) - The Jet electric chain hoist was not inspected for defects on a frequent and periodic basis. (b)Locomotive Component Shop (jib hoist located on east wall) - The 2 ton Budget electric roller chain hoist was not inspected for defects on a frequent and periodic basis. (c)Locomotive Component Shop (east wall) - The 1/2 ton Power Vane Hoist, Chicago Pneumatic Tool Company (Serial # 750316, Type B) was not inspected for defects on a frequent and periodic basis. (d)Small Paint Shop - The 1 Ton Jib mounted chain fall air hoist was not inspected for defects on a frequent and periodic basis. The hoist was rated for 2,000 lbs and the jib was only rated for 685 lbs. (e)Maintenance Shop - The 1 ton Jib mounted hoist was not inspected for defects on a frequent and periodic basis. (f)Maintenance Shop - The 1 ton Jib air hoist was not inspected for defects on a frequent and periodic basis. (g)Machine Shop - The Chicago Pneumatic Tool Company 4,000 lbs jib air chain hoist was not inspected for defects on a frequent and periodic basis. The hoist was rated for 4,000 lbs and the jib was only rated for 1,000 lbs. (h)Machine Shop - The Budget 2 ton jib hoist was not inspected for defects on a frequent and periodic basis. The hoist was rated for 4,000 lbs and the jib was only rated for 1,000 lbs. (i)Machine Shop (located east of the FAB Shop) - The Cyclone 1/2 ton chain fall hoist was not inspected for defects on a frequent and periodic basis. (j)Annex Truck Shop - The jib air hoist (Plant # A-2) was not inspected for defects on a frequent and periodic basis. (k)Annex Truck Shop - The jib air hoist (Plant # B-4) was not inspected for defects on a frequent and periodic basis. (l)Annex Truck and Engine Shop - The ARRO 1 ton jib hoist (Plant # B-1) was not inspected for defects on a frequent and periodic basis.(m)Annex Truck Shop - The 1 ton air hoist (Plant # A-4) was not inspected for defects on a frequent and periodic basis. (n)Annex Engine Shop - The 1 ton air hoist (Plant # C-4) was not inspected for defects on a frequent and periodic basis. (o)Annex Engine Shop - The 1 ton air hoist (Plant # C-5) was not inspected for defects on a frequent and periodic basis. (p)Annex Truck and Engine Shop - The 1 ton air hoist (Plant # C-9) was not inspected on a frequent and periodic basis. (q)Annex Parts Cleaner Area - The 2 ton Reliable hoist (Plant # A-2) was not inspected on a frequent and periodic basis. (r)Annex Engine Rollover Area - The 2 ton hoist (Plant # C-2) was not inspected on a frequent and periodic basis. (s)Annex Engine Rollover Area - The 2 ton hoist (Plant # C-3) was not inspected on a frequent and periodic basis. (t)Annex Engine Rollover Area - The 2 ton hoist (Plant # C-6) was not inspected on a frequent and periodic basis. (u)Annex Engine Rollover Area - The 2 ton hoist (Plant # C-7) was not inspected on a frequent and periodic basis. (v)Annex Engine Build-up Area - The 1 Ton hoist (Plant # D-4) was not inspected on a frequent and periodic basis. (w)Annex Power Assembly Build-up Area - The 1/2 ton hoist (Plant # D-3) was not inspected on a frequent and periodic basis. Note: Abatement certification required for this item. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO INSPECT MATERIAL HANDLING HOIST HOOKS IN ACCORDANCE WITH ANSI B30.10, AND THE HOIST MECHANISM IN ACCORDANCE WITH B30.16 CURRENT EDITION; AS WELL AS FOLLOWING THE MANUFACTURER'S RECOMMENDATIONS. ENSURE THAT STRUCTURAL SUPPORT FOR THE HOIST MEETS OR EXCEEDS THE MAXIMUM CAPACITY OF THE HOIST, WITH APPROPRIATE SAFETY FACTOR."
