Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from possible overhead structure/hoist failure: (a)New/West Shop: On or about 11 February 1998, and at times prior thereto, the hook for the Ingersoll-Rand 2-ton electric hoist was not inspected on a frequent and periodic basis in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and Slings, ASME B30.10a-1996, paragraphs 10-1.2.1.2 and 10-12.1.3 (b) New/West Shop: On or about 11 February 1998, and at times prior thereto, the Ingersoll-Rand 2-ton electric hoist was not inspected on a frequent and periodic basis in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and Slings, ASME B30.16b-1995, paragraphs 16-2.2.1.2 and 16-2.2.1.3. (c)New/West Shop: On or about 11 February 1998, and at times prior thereto, the overhead support structure for the Ingersoll-Rand 2-ton electric hoist was not inspected on a frequent and periodic basis in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Overhead and Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist), ASME B30.17-1992, paragraphs 17-2.1.2 and 17-2.1.3. (d)New/West Shop: On or about 11 February 1998, and at times prior thereto, the screw pin shackle supporting the Ingersoll-Rand 2-ton electric hoist mounted on the bridge truck on the overhead structure was not secured from vibrating loose by an approved method, such as, but not limited to, mousing the screw pen to the shackle or using a bolt cotter pen, in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Overhead and Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist), ASME B30.17-1992, paragraphs 17-2.1.3. (e)New/West Shop: On or about 11 February 1998, and at times prior thereto, the nylon locking nuts were not sufficiently tightened to the bolts of the bridge truck for the Ingersoll-Rand 2-ton electric hoist, in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Overhead and Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist), ASME B30.17-1992, paragraphs 17-2.1.3. (f)New/West Shop: On or about 11 February 1998, and at times prior thereto, the nylon locking nuts were not sufficiently tightened to the bolts of the girder trucks supporting the bridge for the Ingersoll-Rand 2-ton electric hoist, in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Overhead and Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist), ASME B30.17-1992, paragraphs 17-2.1.3. (g)New/West Shop: On or about 11 February 1998, and at times prior thereto, the supporting structure and the Ingersoll-Rand 2-ton electric hoist did not have a record documenting that an appropriate rated load test had been performed, in accordance with the American Society of Mechanical Engineers (ASME) Safety Standards for Overhead and Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist), ASME B30.17-1992, paragraphs 17- 2.2.2. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install, inspect, and maintain overhead structure and hoist in accordance with the American Society of Mechanical Engineers (ASME) B30.10, B30.16 and B.30.17.
