Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a fall hazard a) Work site, front of building - An employee was elevated approximately 14 feet high and was standing on (4) planks that were suspended on the forks of a Lull rough terrain forklift, Model # R622-4TC, SN # 75882 as he was attaching decorative trim. The Pride Lull was parked on the curb with approximately the rear third of the Lull extended into the street. The employee was exposed to the hazard of falling 14 feet to the ground as fall protection was not provided: On of about 05/07/99. ABATEMENT NOTES: Among others, one suitable method for abating this hazardous condition is 1. Utilize only platforms designed for the use of elevating personnel with forklifts. These platforms must meet the requirements set forth in ANSI/ASME B56.6-1987 such as, but not limited to, fall protection for employees. 2. Train all employees about the hazards of falls associated with improper elevating methods and the proper application and use of safety platforms.
