Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing/amputation hazard: a) Plant Area: Employer did not ensure by adequate supervision that correct operating/cleaning procedures were being followed thereby exposing employee(s) to a crushing/amputation hazard. Violation occurred on 2/23/98 "Among other methods, one feasible and acceptable abatement method to correct this hazard is to secure cleaning tools and train and supervise employee(s) in accordance with ANSI Standard B11.3-1973 - Safety requirements for the construction, care, and use of power press brakes, Section 6.1.7"n
