Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of being caught by and drawn into machinery which could result in crushing and/or amputation from reaching into remove debris from the Sunflower and Millet mechanical auger running from the train shed to the bins, while repairing the auger when it becomes jammed. Among other methods, one feasible and acceptable method of abatement is to enforce the existing lockout procedures in the plant by following these essential points: 1) Provide each potentially exposed employee working on the augers with a separate lock and key for each disconnecting means. 2) Thoroughly train all maintenance personnel in all points of the lockout procedure, and continue to update the training. 3) Periodically evaluate the lockout program and strictly enforce all of its provisions.
