Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by and/or crushed: a)On or about 05/24/05, new waterline being installed on Pinnacle and Goodburlet Roads. The hazard of being struck by and/or run over while working and/or moving near heavy equipment. The factory installed mirrors on the right and left side of the Caterpiller excavator, Model 322B- L, had been broken off or removed and had not been replaced prior to the machine being utilized in a forward or reverse motion. Among other methods, one feasible and acceptable abatement method to correct this hazard is to inspect and maintain all equipment as prescribed by the manufacturer to ensure that all standard equipment/safety devices are inspected regularly, properly used, and correctly attached in the prescribed position.
