29 CFR 1918.93(d)(4): Appropriate warning signs or equivalent means were not posted at all means of access to spaces identified as having hazardous, flammable, or oxygen-deficient atmospheres in order to prevent inadvertent employee entry: (a) Employees were exposed to death and serious health effects because hatches providing access into barge storage tanks were not labeled to warn employees of a potentially oxygen-deficient atmosphere.
