Text For Citation: 01 Item/Group: 001 Hazard:
Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by hazards while using hoists in the workplace: a) KARD Welding, Inc. located on 00480 Osterloh Road in Minister, OH: On or about February 16, 2012, the employer did not ensure that the hook on the 25 ton West main bay crane was equipped with a throat latch, exposing employees to struck-by and crushing hazards should fabricated beams or components roll out of the hook under slack conditions. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to comply with the requirements of The American Society of Mechanical Engineers (ASME) B30. 16-2007 "Overhead Hoists (Underhung)", Section 16-1.2.9 "Hooks".
