OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Employees were exposed to a fall hazard in excess of four feet. a) Repack Room, Suction Line - An employee was observed standing on elevated facility piping while painting. The employee was exposed to falls in excess of 16 feet. On or about 2/27/12. b) Repack Room, Suction Line - While standing on elevated facility piping, an employee attached his fall protection to a pipe support not rated for at least 5000 pounds. The employee was exposed to a fall hazard in excess of 16 feet. On or about 2/27/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
