29 CFR 1915.89(c)(2) Energy-isolating device capable of being locked, was not locked by the employer to prevent energization or startup, or the release of hazardous energy, before any servicing is started, unless the employer can demonstrate that the utilization of a tags-plus system will provide full employee protection as set forth in paragraph (c)(6) of this section. a) USCG Polar Star, Vigor Shipyard, Energy isolating devices that were capable of being locked were not locked, including but not limited to Vent fan 2-141-2 control in CHS filter room tag E-060-1 & E-060-2. In Aux 3 Ship Service air #1 breaker 3-133-1 tag E- 258-1, LO Purifier 2a & 2b tag E-361-1 & E561-3.
