29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE): (a) On or about 01/27/2012, at the Patterson-UTI Drilling Company Rig #491 near Keene, ND, employees were exposed to the potential hazard of a boiler malfunction when the employer failed to conduct a hazard assessment of boiler operation and maintenance procedures. Abatement certification and documentation are required for this item (see enclosed "Sample Abatement Certification Letter").
