29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee: a. Site - On January 29, 2026, an employee was seriously injured after falling approximately 15 feet onto frozen ground when the anchorage point of his personal fall arrest system failed when he fell from the elevated location he had been working from.
