29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees or contractor employees were engaged in activities such as servicing and/ or maintenance of equipment: At 1800 Apache Avenue, Hiawatha, Kansas, in north end of production building, employee(s) engaged in cleaning the Roller Miller line #2 was exposed to a crushing hazard. Lockout / tagout procedure was not being utilized to control all energy sources of the machine prior to maintenance work being performed.
