29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) 99 Horseshoe Road, Leola PA - On or about December 15, 2025, the employer had not reported the in-patient hospitalization of an employee to OSHA within the required 24-hour period. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
