29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Excavation located at 4669 Church Road, Cumming, GA 30040: On or about 12/17/2025, the employer exposed employees to cave-in/engulfment hazards while working inside a trench which measured approximately 6-feet, 8 inches deep by 12 foot, 4-inches wide by 16-feet long, in that an adequate cave-in protection system was not provided to a employees engaged in water tap gate valve installation.
