29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) On or about November 10, 2025 at 104 E. Wisconsin Ave. Oconomowoc, WI 53066, the employer did not ensure a safety harness with lanyard attached to the boom or basket was worn by employee(s) working from an aerial lift working. The aerial lift was being utilized to lift and move employees around a worksite to manually tie in metal cables connected on tent rafters in order to secure a festival tent. This exposed employees to an ejection hazard, as well as fall hazard of approximately 26 feet.
