29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about October 15, 2025, the employer did not ensure that machine specific procedures for Conveyor CP88 of the RME standup slam system were utilized prior to service or maintenance taking place. A maintenance employee had performed an inspection on the equipment. The employee removed guarding in the hazardous area prior to isolating the energy sources present.
