29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a) South Unit Area Hallway - On or about June 25, 2025, an employee walking down the hallway checking on patients caught their foot on the buckling up loose floorboards causing the employee the trip and fall and hit their head resulting in injury. The employer did not ensure the walking-working surface was maintained free of hazards associated with the buckling up loose floorboards exposing employees to tripping hazards. b) Hallway near Resident Dining Room - On or about August 31, 2025, an employee walking in the hallway going from the Resident Dining Room to the Supervisor's Office caught their foot on the buckling up loose floorboards causing the employee to trip and fall resulting in an in-patient hospitalization. The employer did not ensure the walking-working surface was maintained free of hazards associated with the buckling up loose floorboards exposing employees to tripping hazards.
