29 CFR 1910.269(d)(2)(iii): Procedures shall be developed, documented, and used for the control of potentially hazardous energy covered by paragraph (d) of this section: On or about August 18, 2025, the host employer failed to ensure that procedures for the control of hazardous energy were strictly adhered to. Technicians (both employees of the company and subcontracted employees) were not protected from hazards created by arc flash/arc blast in that technicians engaged in the group lockout of the PCS 025 (SG4400UD-MV-US, Serial #T2520600152) did not verify that electrical energy had been de-energized, and isolated, prior to commencing maintenance activities.
