29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). On or about July 31, 2025, at the worksite located at 305 E. Spring St., Saint Marys, Ohio: The employer did not ensure an accident prevention program was developed and implemented for the work environment, to include regular inspections of the job site conducted by a competent person. Employees performing tasks associated with temporary shoring of the multi-story building and excavation of the foundation were exposed to struck-by and crushing hazards when performing demolition and construction work.
