29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about July 24, 2025, during the replacement of a water line at a worksite located in front of 311 Merriman, Akron, OH 44313, the employer did not ensure an employee working inside a trench, with a depth of approximately 5 1/2 and 6 1/2 feet, was protected from cave-in hazards by an adequately designed protective system.
