29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: (a) On July 24th, 2025, at 6704 Meadow View Drive South in Fargo, North Dakota, an employee was exposed to fall and collapsing hazards while working from a telehandler basket, 12 feet in length, where the basket was installed length-wise off the forks. The platform exceeded two times the load center distance of the forklift.
