Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 10/15/90, employees were exposed to the hazard of an out-of-control motor vehicle. Employees were located at the rear of a running, driverless, 1978 Ford Refuse Collection Compactor, Model D802B, vehicle identification number D802BVA17992, when the vehicle unexpectedly moved backwards, exposing employees to being crushed between the moving vehicle and dock structure. Among other methods, one feasible and acceptable method to correct this hazard, as per ABSU Z245.1, 1984, American National Standard for Refuse Collecting and Compacting Equipment-Safety Requirements, is to include, in the training of vehicle operations, the following: a) As per Section(s) 6.1(1) and 6.1(2), running vehicles will not be left unattended unless procedures for doing so are followed as set forth by the vehicle manufacturer. b) As per Section 6.1(3), daily, prior to vehicle use, the operation of all safety equipment of the vehicle, to include the parking brake, will be verified to be in safe operating condition.
