29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. On or about July 14, 2025, at the internet tower of the worksite, an employee was utilizing a fall protection system and was not provided training on how to correctly use the equipment required to climb and conduct maintenance on the internet tower.
