29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a) Kard Welding, Inc. On or about July 7, 2025 at the location of 480 Osterloh Rd. Minster, OH 45865: The employer did not ensure open holes between powered rollers of the "Wheelabrator" conveying system were adequately guarded as to prevent employees from stepping or tripping into the holes. Employees handling and monitoring material from atop the frame of the west powered roller were exposed to tripping/falling hazards.
