29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about July 1st, 2025, employees engaged in roofing operations located at 5104 North 210th Ave, Elkhorn, Nebraska 68022 were exposed to fall hazards in that multiple employees were observed working on a residential roof at heights greater than 12 feet without any form of fall protection in place.
