29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence. a) On or about June, 27, 2025, employees working in the Distribution areas were exposed to the hazards associated with untrained power industrial truck operators; the employer did not ensure that each powered industrial truck operator completed a training and evaluation program prior to operating power industrial trucks. The ITW Performance Polymers, a division of Illinois Tool Works Inc., a Delaware Corporation, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(ii), which was contained in OSHA inspection number 1641380, citation number 1, item number 1, and was affirmed as a final order on April 21, 2025, with respect to a workplace located at 30 Endicott Street, Danvers, Massachusetts 01923.
