29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) 301 Gap Road, Ronks, PA 17572 - an employee was exposed to uncontrolled hazardous energy while performing structural improvements on the Car Shop Drop Pit, as a lockout device was not affixed to electrical breaker which controlled the Car Shop Drop Pit's function, on or about June 20, 2025. No abatement certification required.
