Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of fire and explosion since propane valves #1, #2, #3 and #4 were not locked and/or tagged in the closed position while employees were performing maintenance and repair work on the "y" strainger located in the anode casting area. In addition natural gas valve #5 was not locked and/or tagged. On or about February 22, 1989. Among other methods, one feasible and acceptable abatement method to correct this hazard is to develop, document, and implement a lockout/ tagout plan for maintenance and repair work on fuel pipelines which should include but is not limited to the following: 1) Job objectives and involved equipment/process 2) Estimated job duration 3) Personnel and crafts involved 4) Personnel sign-on and off provisions 5) Type number, and location of all energy isolating devices requiring lockout/tagout devices 6) Responsible personnel approvals 7) Start-up provisions
