29 CFR 1910.146(d)(4): The employer did not provide the equipment specified in paragraphs (d)(4)(i)-(d)(4)(ix): On or about June 12, 2025, and at times thereafter, employees engaged in underground water utility work entered underground vault(s) without necessary equipment such as but not limited to; ventilation equipment, lighting equipment, forms of communication, in the following instances below: a) On or about June 12, 2025, where one (1) employee was allowed to enter and work in an underground vault; and b) On or about June 13, 2025, where two (2) employees were allowed to enter and work in an underground vault (Vault (53+60)).
