Text For Citation: 01 Item/Group: 001 Hazard: CONFINED
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Hazard of asphyxiation when employees are working in confined space in the trating cylinders without a confined space entry program in effect. Employees have to enter the cylinders to do maintance to the rails or other portions that requie work where the poles and ties have been treated with penta and cresote under pressure. Among other methods, one feasible and acceptable abatement method to correct this hazars is: (a) Develop and implement an effective confined space entry program at Koppers Company, consisting of as a minimum: (1) planning sessions by qualified persons (2) testing of atmosphere (3) ventilation (4) training the personnel (5) lockout tagout (6) standby people (communication) (7) tools and equipment (8) entry permit (9) continuous monitoring of atmosphere (10) recordkeeping
