29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: On or about June 2, 2025, at an establishment located at 2580 West 9th Avenue in Oshkosh, Wisconsin, employees of Octavio Saucedo dba Octavio's Home Improvement LLC were performing residential roofing activities and were not utilizing a designed anchor point capable of supporting 5000lbs while exposed to fall hazards of over 14 feet 3 inches. There were two employees that had connecting devices connected to a single anchor that was only designed for one employee.
