29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about May 16, 2025, and times prior thereto, employees were utilizing pre-installed residential home attic ladders to diagnose and repair HVAC equipment without performing an inspection on the condition of the ladder before use.
