OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by and caught in-between hazards: a) Worksite, cement culvert: On or about May 1, 2025, employees were releasing lifting hardware from anchor pins from the side of a cement culvert wing wall after it had been landed by a crane. While one of the employees was still in the process of releasing one of the lifting hardware, the crane operator was signaled he could hoist the load line up. As the load line was being raised, the sling attached to the lifting hardware was not fully disengaged, tightened up against the culvert wing wall crushing parts of the body of the employee.
