Located at or near 6050 East Broad Street, Columbus Ohio, 43213: On or about April 30, 2025, the employer failed to provide employees with a safe means of accessing and exiting the ladder for the trench shielding box, as the makeshift platforms guardrails could not withstanding at least 200 pounds of force in any direction along the top edge.
