29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: (a) On the roof of the warehouse, located at 5208 S 36th Ave, Tampa, FL 33619, on or about 4/8/2025, the employer exposed employees to fall hazards, in that covers were not provided over skylights as employees were performing roofing work on an approximately 1/12 pitch roof at heights of approximately 21.5 ft.
