Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a.) On or about April 10, 2025, the employer did not ensure an energy control procedure for the Keystone Actuator Model MRP-088U-K-D000 were developed, documented, and utilized by each authorized employee performing maintenance and testing on the equipment. As a result, an employee suffered an abdominal contusion and liver laceration when the piston shot out of the actuator and struck the employee.
