29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) 500 Madix Rd. Eclectic, AL, Special metals department - Cincinnati Brake Press Model 135 CB2 S/N 52679 ; On or about April 10, 2025 and times prior to, the employer exposed employees to crush-by and amputation hazards when operating the press brake in that the point of operation was not guarded and/or by-passed. Madix, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1620394, citation number 1, item number 1 and was affirmed as a final order on March 29, 2023, with respect to a workplace located at 20984 AL Hwy 9, Goodwater, AL 35072.
