29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a. On or about March 29, 2025, in the warehouse, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices during maintenance or servicing of the Rebar Spiral Machine #1. As a result of the inadvertent start-up of the equipment and/or the release of stored hazardous energy, an employee sustained an amputation to the left-hand pinky finger.
