29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. In the shop area, at 291 Springview Commerce Drive, Debary, Florida: On or about April 8, 2025, the employer exposed employees to amputation hazards, in that procedures were not developed and utilized for the unexpected energization, or start-up of the machines or equipment, while employees conduct maintenance operations.
