Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(7)(iii)(A) Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures: Employee(s) were exposed to amputation hazards while performing maintenance. This was documented on or about April 9, 2025, at 4200 Hwy 30 East, Kearney, Nebraska. Employees were exposed to amputations in that the energy sources were not removed from the Forge Lube Vac Truck Tank prior to removing guards for a belt and pulley. The power cord was not disconnected from the power source which allowed the machine to start while the employee was exposed to an amputation hazard (in-running nip point). Cord and plug devices must be unplugged and the cord maintained under the exclusive control of the person bypassing a guard or performing maintenance per the standard in order to render the machine safe.
