29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) On or about March 20, 2025, at the above addressed jobsite, an employee was lifted out of an excavation approximately six (6) feet eight (8) inches in depth by the tool of a Caterpillar 340 hydraulic excavator Serial No. EFH30012, thereby exposing the employee to fall and struck by hazards. b) On or about March 20, 2025, at the above addressed jobsite, employees conducting storm sewer installation in an excavation approximately six (6) feet in depth did not have a safe means of egress.
