29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: a) Car Wash Tunnel - On or about 2/26/2025, and at times prior, the employer exposed employees to entrapment, struck-by and caught-in hazards, in that the employer failed to ensure employees affixed LO/TO devices at the electrical panel of the wrap-around machine during cleaning operations.
